Filling out clean break order and pension sharing annex

I have agreed a percentage of my ex husbands pension. The judge said I need to do a clean break order and also complete a pension sharing annex. Once the pension sharing annex is completed I have to sign, send to ex to sign, send to Pension provider to sign then send to court to be sealed. I need help doing these forms/what forms are required.

You should ask a solicitor to do it. There are no standard forms for this. Even if there were you would be as unwise to do it yourself as it would be unwise to draft your own will.
